First Visit and Consultation Appointment

In order to ensure that you receive the best quality of care, your initial appointment will consist of a consultation appointment with your surgeon explaining the diagnosis and treatment options. A complex medical history or treatment plan may also require a second consultation appointment prior to initiating your treatment.

Please assist us by providing the following information at the time of your consultation:

  • The surgical referral note from your Dentist or physician.
  • Any x-rays if applicable.
  • A list of medications you are presently taking and the name and telephone number of your pharmacy.
  • If you have dental insurance, please bring your insurance information (including service provider, certificate number and policy number) with you to the consultation. This will save time and allow us to help you process any claims.

Important: All patients under 18 years of age must be accompanied by a parent or guardian at the consultation visit. However, for complex surgical consultations such as Orthognathic surgery, reconstructive surgery, or other forms of jaw surgery, we strongly recommend patients under 21 years of age to be accompanied by their parents or legal guardians.

A preoperative consultation and physical examination is mandatory for patients undergoing IV sedation or general anesthesia for surgery. PLEASE HAVE NOTHING TO EAT OR DRINK (NOT EVEN WATER) AT LEAST SIX HOURS PRIOR TO YOUR SURGERY. You will also need a responsible adult to drive you home.

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office prior to your appointment, preferably via email (xray@yorkvilleoralsurgery.com). If there is not enough time, please pick them up and bring them to our office. Please understand that additional x-rays may be required and can be taken at our office in order to fully assess your treatment needs.
