Written consent to share information is obtained in order to provide the best medical and oral health care. It is sometimes necessary to discuss or share diagnostic materials and findings pertaining to your medical history or oral health with your medical physicians or other dental professionals including your referring dental office.
Written consent to share information is also required in order to assist in the processing of dental insurance claims and electronic submissions of dental insurance claims and pre-determinations.
